Job Description

Are you looking for a new opportunity in healthcare that prioritizes the patient and their wellbeing? This may be the opportunity you have been looking for! We are in search of an Inpatient Care Coordinator to join our team!

Day To Day Responsibilities

  • Review daily hospital census and faxes and enter new admissions into EHR system
  • Notify inpatient case manager of admissions
  • Manage inpatient call queue & voicemails; respond to messages within 24 hours
  • Notify discharge coordinator of all discharges
  • Discharge planning call hospitals, search for home health, SNF, DME or specialists and authorize services by case manager
  • Appointment scheduling for PCP and specialist
  • Set up temporary contracts with new providers

Qualifications

  • High School Diploma or GED
  • Previous experience in customer service or medical experience (preferred)
  • Medical Assistant Certification (a plus!)
