Job Description


We are seeking a compassionate and dedicated Care Coordinator to join our team. The Care Coordinator will play a key role in assessing clients' needs, developing personalized care plans, and coordinating healthcare services to ensure optimal outcomes for our clients.


Responsibilities:

  • Develop individualized care plans in collaboration with interdisciplinary teams
  • Conduct comprehensive assessments of clients' physical, emotional, and social needs
  • Coordinate healthcare services, including medical appointments, therapy sessions, and home care visits
  • Serve as a central point of contact for clients, families, and healthcare providers
  • Advocate for clients' rights and preferences to ensure their needs are met
  • Monitor clients progress and adjust care plans as needed to optimize outcomes
  • Maintain accurate records of client assessments, care plans, and interventions
  • Provide education and support to clients and families regarding their health conditions and available resources
  • Participate in quality improvement initiatives to enhance the delivery of care services

Qualification:

  • 2+ years of Care coordinator experience
  • Have a valid DL
  • Bachelor s degree in social work/ psychology
